Flexible Locum Social Worker Position – Make a Meaningful Impact on Your Terms
Empowered to Thrive, a dynamic private practice based at Faircape Health Tokai Estate, is seeking a skilled Locum/Part-Time Social Worker to join our team. This is a unique opportunity for an experienced professional who is passionate about holistic, patient-centered care and values the flexibility to shape their own schedule.
At Empowered to Thrive, we support individuals navigating complex medical recoveries—including Neurological, Orthopedic, Surgical, and Respite care—by offering critical psychosocial services that promote well-being and safe discharge planning. Our social workers play a vital role in supporting patients and families through counseling, mediation, case coordination, and stress-reducing interventions to ensure optimal engagement in the rehabilitation journey.

Responsibilities:
- Assess Care needs
- Assist families to implement recommended care to ensure safe discharges are in place.
- Connect patients to relevant resources such as care agencies, frail cares, day clinics, palliative care resources etc.
- Make referrals to appropriate service providers such as psychologists, Department of Social Development etc.
- Provide short term counseling
- Provide mediation to patients and families
- Provide practical assistance to patients to reduce stressors in order to participate in their rehabilitation.
What We’re Looking For
- SACSSP Registration (current and valid)
- Professional Indemnity Insurance
- Training and experience in Palliative Care (essential)
- Additional experience in Oncology, Curatorship, or Geriatric/Frail Care (advantageous)
- Proven ability in Hospital Social Work and Case Management
- Strong report writing and time management skills
- Comfortable working flexible hours within normal business times
- Willingness to travel for outpatient consultations
- Ability to collaborate effectively with an interdisciplinary care team
